PHILOSOPHY OF PATIENT CARE
Our goal is to provide specialized orthopedic manual physical therapy treatment that is specifically designed for the patient’s condition. However, Individualized treatment plan can only be established after conducting a thorough and precise clinical examination and evaluation. We believe that treatment is the easiest part of the whole cycle of patient care. Furthermore, we believe that the evaluation and reevaluation process is the key to unlocking the mystery behind the patient’s pain or dysfunction/impairment.

Why SMPT? SMPT recognizes that a patient’s condition may arise from multiple components of dysfunction, thus no single form of manual therapy approach in itself may be sufficient to address such condition. SMPT has position itself to have the ability to evaluate and treat even the most difficult and challenging conditions. In order to do that, Daniel has undergone extensive training with the most advanced manual therapy institutions. What is manual Physical Therapy?
A clinical approach utilizing skilled, specific hands-on techniques, including but not limited to manipulation/mobilization, used by physical therapists to diagnose and treat soft tissue and joint structures for the purpose of modulating pain, increasing range of motion, reducing or eliminating soft tissue inflammation, inducing relaxation, improving contractile and not contractile repair, extensibility and or stability, facilitating improvement and function.
What is mobilization/manipulation? a manual therapy technique comprised of a continuum of skilled passive movements to joints and/or related soft tissues that are applied at varying speeds and amplitudes, including a small amplitude/high velocity therapeutic movement." Mobilization and manipulation are an integral part of the direct interventions listed under the Guide to Physical Therapy Practice under the section entitled “ Manual Therapy Techniques.
What is Orthopedic Manual Therapy? It is a specialized area of physiotherapy or physical therapy for the management of neuro-muscular conditions, based on clinical reasoning, usually highly specific treatment approach including manual techniques and therapeutic exercises. OMT also encompasses, and is driven by, the available scientific and clinical evidence and biophysical framework of each individual patient.
What is soft tissue mobilization / myofascial release? The forceful passive movement of the musculo-skeletal elements through its restrictive direction(s) beginning with its most superficial layers nad progressing into depth while taking into account its relatrionship to the joints concerned.
What is Functional mobilization? This is the process of combining the principles of joint mobilization and/or soft tissue mobilization with functional movements.
What is HVLAT? It stands for High Velocity-Low Amplitude Technique, also known in physical therapy as thrust, mobilization with impulse, grade V mobilization. Grade V mobilization is a PT term that was first used in the Mobilization the Spine by Greg Grieve 3rd edition.
SMPT utilizes short or minimal level manipulation. It is a manipulation that focus at one specific point of barrier or tension while everything above and below the joint is free to move and are not involved in the process. This is a non-torsional, mid-range, subtle manipulation that offers minimal damage or no pain or discomfort at all. As a rule, SMPT utilizes manipulation technique as a last resort, especially if and when clinical judgment dictates that other forms of manual physical therapy approach that utilizes less force are as effective as well.
What is muscle energy technique? “Muscle energy technique is a manual medicine treatment procedure that involves the voluntary contracture of patient muscles in a precisely controlled direction, at varying levels of intensity, against a distinctly executed counterforce applied by the operator. Muscle energy is classified as an active technique in which the patient provides the corrective force. The activating force is classified as intrinsic. The patient is responsible for the dosage applied.”
What is trigger point? “Travell and Simons define trigger point as a highly irritable localized spot of exquisite tenderness in a nodule in a palpable taut band of muscle tissue. The first part of that very exacting definition is just means that a trigger point hurts a lot when you push on it. When a trigger point is active enough, you are likely to startle, wince, and pull away. This is called giving the “jump sign”.
What is trigger point deactivation? “A number of methods exists for the deactivation of such trigger points, ranging form the use of pharmacological agents such as Novocain or xylocaine, to coolant sprays and acupuncture techniques”. “Trigger point deactivation may be achieved by ( Chaitow & DeLany 2000, Kuchera 1997, Travell and Simon 1992):

Why integrated or eclectic?
It is the belief of SMPT that there is no single approach or technique that will best manage a patient dysfunction, impairment, or pain. Treatment approach can even change from any given point during the course of the therapy, considering the complexity of the etiology of most of patient’s condition. A lot of clinical studies were done in order to identify the so called “magic pill” that can be used as the best treatment approach for the patient’s condition. I firmly believe that all these randomized controlled studies are of great benefit to the manual therapists in particular, and to the growth of the physical therapy profession in general. However these clinical studies can only serve as a guide at best. Nothing can substitute for an astute clinical mind of a practitioner who strives to do his best to gather and understand as much pertinent or relevant information not only during the process of initial examination, but also during the whole course of the treatment episode. SMPT however emphasizes that a practitioner has to have the clinical knowledge, experience, and training in order to have much better understanding and interpretation of the clinical data at hand. It is to this end that SMPT imposes upon itself to continually learn new methods or approach that can be used individually or in combination of any of its already existing array of manual therapy skills/techniques. SMPT also believes in evidenced based practice of physical therapy; however SMPT does not deny its patients from the merits or benefits that may be gained from new or evolving manual physical therapy concepts. However its use or application does not involved any diluted sense of application of the standards to which SMPT stands for.
Why Manual Physical Therapy?
The fact is that a lot of the treatments that are rendered by practitioners are based mostly in diagnosis reached upon a result of a radiologic examination. However in many cases of musculoskeletal conditions, relying on positive radiologic finding as a basis of rendering care may not be for the best interest of the patient. MRI, CT scan, and all other radiologic exam may depict some abnormalities of structure. However this does not necessarily mean that such abnormalities are the cause of the patient’s musculoskeletal impairment or dysfunction. This is what is known to be called false positive findings. It is for this reason that some patients after surgery still have the same symptoms as that prior to surgery. Manual Physical Therapy combines the optimal anatomical and biomechanical knowledge in order to come up with a more thorough and detailed examination. All objective findings, including radiologic reports and all other pertinent information, are taken into account in order to come up with a more conclusive interpretation of the patient’s condition. Only then can a meaningful and individualized treatment plan can be established.
