INSURANCE INFORMATION
Physical Therapy is a covered benefit under most insurance plans. As a courtesy to our patients, insurance is billed directly to the carrier. Benefits, deductibles and co-payments vary greatly from plan to plan and will be discussed on an individual basis. We encourage patients to contact their insurance carriers personally to confirm their benefit information.
At the time of your first appointment, you must bring all applicable insurance cards, forms, and claim numbers necessary for insurance billing.
Please be aware, verification of benefits is not always a guarantee of payment. Physical therapy claims will be submitted on behalf of the patient by SMPT. You are responsible for payment of services rendered to you regardless of coverage status.
Any change in insurance coverage, employer, or home address must be reported to the office staff.
Routine questions about costs or payment plans can be directed to our front desk personnel.
SMPT will verify your insurance benefits with the appropriate carrier. Benefit information will be provided to you prior to your first appointment or at the time of your first appointment. Please note that the faster we receive your insurance information, the quicker we can inform you of your benefits. In some cases, it is necessary for us to ask for your assistance in obtaining information from your insurance carrier.
WORKERS COMPENSATION / NO FAULT: It is very important that, with workers compensation or motor vehicle accidents, that we receive a copy of your health insurance card as a back up in case the previous insurance reaches its maximum payment or is denied.
SECONDARY INSURANCE: If you have a secondary insurance, we will then file with that insurance provided we have been given all required information.
NO SECONDARY INSURANCE: If there is no secondary insurance to file, we will transfer the balance to your responsibility and you will receive a statement sometime during the first week of the month. (Statements are sent out on a monthly basis until the account is paid in full.)
CO-PAY: If you have a co-pay, that amount is due at the time of service. If you have coinsurance (i.e. 20% due from you and 80% due from your insurance), we would like you to make small incremental payments each visit.
