SCHEDULING FOR APPOINTMENT:
Please contact our office in person or by phone to arrange for your first appointment.
All new appointments are scheduled within 24 to 48 hours. Ongoing appointments are scheduled according to the patient’s availability. Sunday and Weekday’s evening appointments are available in order to accommodate the patient’s work or school schedule. Tuesday and Thursday morning schedules are limited by appointment only and that strict scheduling are enforced during this period.

YOUR FIRST APPOINTMENT:
At your first appointment your condition will be evaluated by the physical therapist. The therapist will ask you questions regarding your symptoms and history. Please let the therapist know if there are any questions that you do not understand or if you have any special concerns regarding manual physical therapy.
The therapist will then take objective measurements of movement, strength, reflexes, sensation, gait and balance. It is necessary to visualize the areas needing treatment. On your first visit please bring or wear loose fitting comfortable clothes such as shorts, tank tops or T-shirts. A hospital gown can be provided if necessary. The first visit will likely take about 1 to 1 1/2 hours.
A physical therapy program will be designed based on the individual needs of the patient. Specific treatments may be prescribed by your physician but it is the physical therapist’s responsibility to evaluate the patient to determine the level, intensity, and progression of the physical therapy program. Any changes in the treatment plan are done in consultation with your physician.
Please print and fill out the following forms prior to coming to the clinic. This will speed up the process of the examination that is done prior to your first treatment session. You may choose to fax it to 718-332-2458 or you may bring it with you during your first visit. Should you have problems filling up some parts of the form, we will assist you in completing that part of the form as you come in for your first visit.
